Grey County Long-Term Care homes reach milestone in Clinical Pathways initiative

Grey County's three long-term care homes — Grey Gables, Lee Manor and Rockwood Terrace have reached the halfway point of a three-year initiative to implement the Registered Nurses Association of Ontario (RNAO)'s Clinical Pathways Program, aimed at improving resident care through proven best practices.

The program provides team members with clear, research-based tools and guidelines to deliver consistent, high-quality care for residents. By integrating Clinical Pathways into electronic medical records, the homes are improving documentation and ensuring every resident receives the same standard of excellent care.

Team members have been rolling out foundational tools since April 30, 2024. These include admission assessment, delirium assessment, resident and family-centred care, fall prevention, and assessment and management of pain. Across all three homes, team members have been working to make these practices part of everyday routines.

On October 30, the initiative reached a significant milestone with the rollout of two additional pathways: the Palliative and End-of-Life Clinical Pathways. These pathways ensure that residents and their families receive compassionate, personalized care that honours their values, wishes and needs during end-of-life care.

"Clinical Pathways reflects Grey County’s commitment to continuous improvement and excellence in care," said Denna Leach, Long-Term Care Clinical Specialist. "By embedding best practices into daily routines, we are creating a more consistent and supportive environment for residents and their families.”

Jennifer Cornell, Director of Long-Term Care and Senior Services, commended the ongoing efforts. "We're proud of our team's commitment to Colour it with Clinical Pathways and we are building on our dedication to person-centred care. Reaching this halfway point shows our team's commitment to providing the best possible care for residents."

The implementation of Clinical Pathways is a collaborative effort involving dedicated care team members, clinical specialists and leadership teams, with ongoing training and evaluation to ensure success. Grey County remains dedicated to fostering a culture of quality, safety, and dignity in long-term care, and to Colouring It Your Way for every resident and family.
